Chiropractic appointment - 60 min
The first chiropractic appointment consists of discussing your case (and exams), assessment, diagnosis, treatment plan and the first treatment.

Follow-up appointments consist of treatment according to your treatment plan, with sufficient time to discuss your progress and any new complaints or symptoms that might come up.

48h cancellation policy
If you can’t make it to your appointment please cancel at least 48h before your appointment to avoid being charged and so that your time slot can still be used for someone else who needs it 🙂Â

What to bring?
– wear flexible clothingÂ
– water
– a big towel (optional)
– results and images of exams (x-rays, MRI, CT, ultrasound, blood labs etc)
Before your first appoinment you will receive an intake form to fill out.Â
Please e-mail this to chiropractic@consciousli.com or bring it with you.Â
If you cannot fill it out beforehand, please arrive 15 minutes earlier to your appointment to fill it out in the clinic 🙂
